How does Dr. Hacker’s Q switched Alex Trivantage laser work?
Dr. Hacker uses lasers to produce a very short burst of intense but gentle laser light. This light Dr. Hacker generates using his Alex Trivantage Q switched laser has the proper wavelength (“color”) to be absorbed by pigment. (Pigment is any “colored” substance.) Absorption causes “photothermolysis”, or heat damage, preferentially to the implanted tattoo or skin discolorations pigment without significantly damaging surrounding skin tissue. Dr. Steven Hacker’s laser treatment usually leaves other adjacent tissue relatively intact, so that healing occurs with normal pigmentation, or skin color in most patients.
How many sessions and how often will Dr. Hacker use his Q switched Alex Trivantage laser to remove tattoos?
Dr. Hacker plans each tattoo laser removal treatment usually every 4-8 weeks or more apart. Waiting 4-8 weeks gives your body the time needed to “carry away” the laser damaged tattoo or skin discolorations pigment, which you see as fading of the tattoo or skin discolorations. Because this fading process is somewhat individual and tattoo or skin discolorations ink is not standardized, and the tattoo or skin discoloration’s depth depends on how long ago and how deep it was applied, we cannot guarantee 100% complete removal of all tattoo or skin discolorations. In general for tattoos, amateur tattoo respond best (and more quickly) in 3 – 8 sessions. Professional tattoo may take 6 - 12 treatments, or more. The R20 method is a newer treatment approach to try and treat a tattoo in less treatment sessions. The doctor can discuss in more detail this option.

The R20 protocol
The “R20 protocol” or “R20 method” for tattoo removal involves the same high-tech laser devices that were previously used in dermatology clinics, but they are being utilized in a new way for more effective, accelerated tattoo removal. With the R20 protocol, a dermatology laser surgeon administers a series of treatments over the inked area with a laser device. These treatments or “passes” are spaced twenty minutes apart during a single office visit, rather than administered as single treatments in separate visits spaced over many months.
Advantages over Other Treatments For conventional laser treatments, dermatologists used a laser device to make a single pass over the tattoo at each office visit. It took several months before a tattoo would begin to fade. With the new R20 method, more ink is broken down and lifted per visit. Instead of investing the 12 months (or more) that it used to take to complete an entire series of laser treatments, the R20 method means you may be done with treatment in less months, and in come cases depending upon the tattoo type, may be as short as 8 months. (*no guarantees as depends on tattoo technique and ink).

Are there alternatives to Dr. Hacker using a Q switched Alex Trivantage laser for tattoo removal?
In the past, tattoos have been removed by salabrasion, excision (cutting), or chemical peels. Also there are multiple other types of lasers on the market that also treat tattoos in addition to the Q switched ND YAG Alex Trivantage laser that Dr. Hacker uses in his office.
Dr. Hacker may determine upon consultation that you may not be a candidate for tattoo or skin discolorations laser therapy?
Certain tattoo colors and inks respond better than others. Also, Dr. Hacker may explain that his laser is less effective in moderately dark or black skin or for unknown reasons.
What are the risks when Dr. Hacker uses his laser for tattoo removal?
It should be expected for Dr. Hacker’s patients to have “pin point” bleeding points, scabs, swelling or blisters immediately after each treatment (and later, crusts or scabs). Swelling for a day or two or more after treatment is not uncommon. Our patients may also experience some temporary discoloration or reddening of the skin near the treatment site(s).
Depending on your underlying skin pigment, yo may experience lightening of the skin for up to a few months after treatment, normal pigmentation usually returns. Occasionally, removing the tattoo or skin discolorations “unmasks” mild scarring or lightened skin already present, but not readily visible before tattoo or skin discolorations removal, that was due to the original tattoo or skin discoloration process, itself. Also, it is important to know that Dr. Hacker will explain the risk of permanent scarring can happen with any laser procedure. Dr. Hacker wants all of his patients to understand that laser treatment is not 100% or perfect and thus some residual pigment may remain after a series of treatments.
Will Dr. Hacker’s treatment hurt?
Dr. Hacker will offer the optional use local anesthesia to make the procedure painless and because the laser causes mild discomfort. Patients describe the feeling of a laser pulse “the slight sting of a mosquito bite.” Most patients tolerate laser treatment with no need for anesthesia.
If you need some, the doctor will discuss the right kind to use (e.g. an injection of “local”, or applying a cream one hour before treatment).
How long will Dr. Hacker’s laser treatment last?
The time your laser treatment will take depends on the size and number of tattoo or skin discolorations and how well you tolerate the treatment. Most sessions last from 5-30 minutes.
What safety measures are needed during laser treatment?
Since the laser produces a very bright beam of light, we make sure everyone must wear protective eyewear while the laser is in use. If there is hair near the treatment area, we recommend you shave this area on the morning of the procedure.
Other lasers for Tattoo removal
Some of the most common types of lasers used for tattoo removal are:
Is a picosecond aesthetic laser. This technology delivers ultra-short pulse bursts of energy to the skin in trillionths of a second. Picosecond pulse width is 100 times shorter than nanosecond technology, enabling unmatched photomechanical impact for better clearance with fewer treatments and less fluence. This is the newest laser for tattoo removal but also the most expensive per treatment with the same results as the Alex Trivantage but in fewer treatments but overall much increased cost. So, the same result can be achieved for half the price with Trivantage.
• Q-Switched Ruby Laser:
This was the first laser used for tattoo removal. The light wavelength of 694nm makes this useful for green tattoos (the most difficult to remove). Although special care must be taken when treating someone with dark skin, this laser is still very useful for removal.
• Q-Switched Nd:
YAG Laser. Introduced in 1991, the neodymium: yttrium-aluminum-garnet laser became the most heavily-used laser in the tattoo removal industry. This laser can operate alone at a wavelength of 1064nm (effective for removal of dark blue or black ink) or with a frequency-doubling crystal to shorten the wavelength to 532nm (effective for removal of red or orange ink).
• Q-switched Alexandrite Laser:
This laser, introduced in 1992, operates at a wavelength of 755nm, which resides between those of the ruby and neodymium: YAG lasers. It is excellent for the removal of green and blue-black tattoos.
All of these lasers are highly useful for tattoo removal, but as you can see, certain lasers more effectively remove certain colors.
